What the leaflets say
None of the four medicines carries an alcohol ban. The NHS tirzepatide page says you can drink alcohol, and adds that it may make nausea and other side effects worse. The Wegovy leaflet says nothing about alcohol beyond the general advice. The UK limit for everyone is the same: 14 units a week, spread over three or more days. A 175ml glass of wine is about 2.3 units and a pint of lager about 2.3 to 3.
Why it hits harder
- Less food in the stomach. Alcohol absorbs faster on an empty stomach, and your stomach is often close to empty now.
- A smaller body. As weight drops, the same drink gives a higher blood alcohol level.
- Less fluid. Most people on the medicines drink less water, so the dehydration from alcohol starts from a lower base.
- A slow stomach. The alcohol sits for longer, which some people find makes them feel worse for longer.
The practical result is that two drinks can feel like four, and the morning after brings the medicine's nausea and a hangover at the same time.
Many people lose the taste for it
A common report from people on GLP-1 medicines is that they want alcohol less. Small trials of semaglutide in people with alcohol use disorder have found reduced drinking, and larger studies are running. It is early. Nobody should take a weight loss medicine to treat drinking. But if you find a glass of wine no longer appeals, that is a known effect and not a fault.
Blood sugar
The weight loss medicines on their own do not cause low blood sugar. If you also take insulin or a sulfonylurea tablet such as gliclazide for diabetes, alcohol raises the risk of a hypo, and the medicine adds to it. Eat when you drink and keep glucose tablets to hand. People without diabetes do not need to worry about this one.
The pancreas warning
Every leaflet lists pancreatitis as a rare but serious side effect. Heavy drinking is also a cause of pancreatitis. The two together are a bad mix. If you drink more than the UK limit, say so on the pharmacy consultation form, since a prescriber may decide the medicine is not right for you, and that is in your interest. Severe stomach pain that spreads to the back, with or without vomiting, needs urgent help whatever you have had to drink.
Calories
Alcohol is 7 calories a gram. A large glass of wine is about 230 calories, a pint of lager about 220, and a double gin and slimline tonic about 100. On a day when you are eating 1,200 calories, three drinks are a third of it with no protein or fibre attached. The Drinkaware unit and calorie calculator does the sums.
If you do drink
- Eat something with protein first, even a boiled egg or some yoghurt.
- A glass of water for every drink.
- Stop one drink earlier than you used to.
- Avoid drinking on injection day and the day after, when nausea is at its peak.
- Skip the fizz. Prosecco, cider and mixers add gas to a stomach that already holds onto it.
